Background Check Form

The final stage in the process for consideration to be on the Sports and Music Camp staff is an authorized background check. The Sports and Music Camp policy requires that we conduct a background check annually for all counseling staff, and at least every three years for all other staff.  

In order for us to conduct a background check the form linked below must be completed and signed and either mailed, e-mailed, or faxed using the contact information below. Please complete this form in a timely manner. If we do not receive this form we will not be able to include you on our staff.

Please Note - The form must be hand-signed, so the only acceptable forms of transmission are:  a mailed physical copy; a scanned copy of the original that is emailed; or a faxed copy.

All counselors must complete this form each year.  Non-counseling staff may not need a background check this year if we conducted one within the past 3 years; however, we recommend submitting this form regardless, so that we have it in our files.

Background Check - FAQ's

The attached document includes the full disclosure of our use for the background check and your rights under the Fair Credit Reporting Act, but the following FAQ’s may be helpful.

Is a criminal background check really necessary?

  • Sadly, the culture in which we live dictates that we must take drastic preventive measures to protect the children and youth whose care we’ve been entrusted with. Furthermore, there are litigation issues: a camp that hasn’t performed background checks faces the risk of catastrophic financial penalties in the event that an allegation of abuse within the camp leads to a conviction or out-of-court settlement.

Who needs to have a background check performed?

  • Any camp employee or volunteer who will have direct contact with any child or youth (less than 18 years of age) during camp-related events.

I’m concerned about confidentiality and privacy issues. Who will see my application and more importantly, who will see the results of my background check?

  • All information will be kept confidential and in a secure filing cabinet. Only the Directors will have access to the files, and even then on a very limited, need-to-know basis.

What will the background check consist of?

  • The background check will consist of a search of the Dept. of Public Safety, the Dept. of Corrections, the Administrative Office of the Courts, the Bureau of Criminal Apprehension, the Sexual Offenders Registry, and the Dept. of Criminal Justice files and records, where available. The search will also include Social Security Number Verification, which provides the name, date issued and state, as well as best available address history.

Why do I have to list my Social Security Number (SSN)?

  • A couple of reasons. The first is that your SSN is the primary key for all databases affiliated with a background check. The second reason is that your SSN is used to verify the following crucial information:

-Whether the SSN is validly issued
-To whom SSN belongs
-In which year and state the SSN was issued
-The current and other known or previous addresses to that SSN

The SSN Verification process can reveal that the number belongs to another individual, has more than one name associated with it (i.e., produces other aliases) including maiden and divorced names, is associated with fraud, is not a validly issued SSN, belongs to a deceased person, produces additional addresses not stated by the application/request form, or if that individual has other SSN’s. Anyone can give a false identification by using another person’s Social Security number. This search ensures that the individual is who they say they are.

What criteria from the background check will determine whether or not someone is approved for work at camp? Who makes that decision?

  • The background check will cite felony and misdemeanor convictions listed in the National Criminal Database Search. The most obvious reason that someone is denied the opportunity to work at camp is any prior conviction pertaining to an offense against a child. It’s quite unlikely, though, that a person with such a conviction would even submit an application. Misdemeanor convictions, and even some felony convictions, would have little or no negative impact in determining a person’s worthiness as a camp employee. To put it another way, the only determining factor in evaluating someone’s criminal record is the likelihood of him or her posing a threat to our young people. In the event a felony conviction turns up on the report, the Directors will decide whether or not to approve the applicant for work with children or youth, and may desire to interview the potential volunteer before making a final decision. If we have a concern about a prior conviction we may, in confidence, ask you about it in a follow-up interview, but even that does not necessarily preclude you from being on staff. Additionally, if you have concerns about revealing information from your past you may choose to speak to the Directors prior to submitting an application.
